Definition

Isovaleric acidemia (IVA) is an organic acidemia caused by biallelic pathogenic variants in the IVD gene, resulting in deficiency of the mitochondrial enzyme isovaleryl-CoA dehydrogenase. Enzyme deficiency leads to accumulation of isovaleryl-CoA derivatives; the characteristic "sweaty feet/cheese" odor is an important diagnostic clue. In the acute neonatal form, encephalopathy with vomiting and lethargy can progress to coma.
