Definition

Biallelic pathogenic variants in the STX11 gene impair syntaxin-11, a protein involved in granule exocytosis by cytotoxic T lymphocytes and NK cells, causing familial HLH type 4 (FHL4). It presents with prolonged high fever, cytopenias, and hepatosplenomegaly; FHL4 generally has a milder course than the other FHL subtypes.
