Microsatellite Instability (MSI)

Definition

MSI is an indicator of replication errors accumulating at microsatellite regions as a result of dysfunction in the DNA mismatch repair (MMR) system (MLH1, MSH2, MSH6, PMS2). High MSI (MSI-H) status is associated with a high tumor mutational burden and predicts response to PD-1 inhibitors such as pembrolizumab; MSI-H is found in about 15% of colorectal cancers.

Inheritance

Both are possible and the distinction is clinically important, a subset of MSI-H cases (about 3% in colorectal cancer) result from hereditary MMR gene mutations related to Lynch syndrome, while the majority (about 12% in colorectal cancer) are due to sporadic/somatic MLH1 promoter hypermethylation.
