Azathioprine, mercaptopurine and thioguanine are inactivated by TPMT (thiopurine methyltransferase); NUDT15 prevents the cytotoxic metabolite from being incorporated into DNA. Patients carrying reduced-function or loss-of-function alleles in TPMT or NUDT15 are at risk of severe myelosuppression at standard doses; NUDT15 loss-of-function alleles are more common in people of Asian and Hispanic ancestry.
