Definition

CYP2C19 catalyzes the bioactivation of the antiplatelet prodrug clopidogrel into its active metabolite. In poor/intermediate metabolizers carrying loss-of-function alleles (*2, *3), formation of the active metabolite is reduced and platelet inhibition is insufficient; this increases the risk of serious cardiovascular events in patients with acute coronary syndrome and those undergoing PCI. The 2022 CPIC update strengthened the recommendation for intermediate metabolizers.
